Position: Part Time Care Manager Location: San Diego, CA Details: Lifeline Healthcare is a growing Care Management, Homecare and Placement company serving seniors in San Diego, looking for a part time Care Manager. The Care Manager position interacts with all levels of personnel as well as clients. The position requires the individual to possess supervisory skills, leadership skills, planning and organizational skills, as well as the ability to train others. Under the direction of the Clinical Director, the Care Manager is responsible for planning, coordinating and implementing the treatment planned for each client. The Care Manager works in unity with the community or institution to assure the highest quality of care for the older adult through education, advocacy and standards of practice.
Candidates must be detail oriented and possess excellent follow through and problem solving skills. Candidates will also be required to have reliable transportation, as the position requires the Care Manager to make client visits etc.
