Geriatric Care Manager
A Geriatric Care Manager is a degreed professional with a specialized body of knowledge and experience related to elder care issues. Geriatric Care Managers work privately with older adults and their families to create a plan of care, which meets the needs of your loved one and the family as a whole. They will meet with you to discuss your concerns that may include issues such as medications, physician interaction, memory concerns, behavioral or mood changes, in-home safety, driving impairment, and financial interests. They will meet with you to discuss your concerns, and assist in the development of a plan of care which is solution oriented in regards to you and your loved ones overall well-being. This assists you in making the right choices.
